Subscription Claims Information

This form is provided to assist members, institutions, and nonmember individuals who have a problem with their subscription.

To submit your claim electronically, please fill out the form below and click on the submit button at the bottom of this page when you are done. If you prefer to submit your claim by fax or mail, please print out the PDF form and send it to the Subscriptions Department.

If you use the services of an agent, send your claim to the agency.
