This form is to be used to submit comments to the Board of Professional Affairs' Committee on Professional Practice and Standards (COPPS) on the current Guidelines on Hospital Privileges: Credentialing and Bylaws.

As stated in the announcement, COPPS seeks comment on the proposed document from interested parties, and especially on the importance, relevance, and adequacy of the prospective text. Comments regarding the applicability of these guidelines for specific practice settings and specialties are desired.
